WASHINGTON, May 12, 2017– Valley Innovative Services, a Pearl, Miss. establishment, is recalling approximately 130,071 pounds of meat and poultry frozen entrée products which included a non-meat ingredient (waffles) that was recalled due to Listeria monocytogenes, the U.S. Department of Agriculture’s Food Safety and Inspection Service (FSIS) announced today.
The heat-treated, not fully cooked, not shelf stable chicken and waffle, turkey ham & waffle, and oatmeal with waffle and sausage patty frozen entrée items were produced and packaged from May 2016 through May 2017. The following products are subject to recall: [View Labels (PDF Only)]
  • 11.40-oz three compartment tray packages containing “Traditions Creamy Chicken Patty & Waffle with Four Seasons Vegetables and Cabbage” with the following production dates: 6/6/2016, 7/13/2016, 8/15/2016, 10/12/2016, 12/21/2016 and 4/11/2017; and the following sell-by dates: 6/6/2017, 7/13/2017, 8/15/2017, 10/12/2017, 12/21/2017 and 4/11/2018.

  • 7.40-oz three compartment tray packages containing “Traditions Oatmeal with Waffle and Sausage Patty” with a production date of 10/14/2016 and a sell-by date of 10/14/2017.
The products subject to recall bear establishment number “EST. 18350” on the packaging label. These items were shipped to institutions nationwide.
The problem was discovered when Valley Innovative Services was notified by their waffle supplier that the waffle product used in the frozen entrée products was recalled due to potential Lm contamination.
There have been no confirmed reports of adverse reactions due to consumption of these products.
Consumption of food contaminated with L. monocytogenes can cause listeriosis, a serious infection that primarily affects older adults, persons with weakened immune systems, and pregnant women and their newborns. Less commonly, persons outside these risk groups are affected.
Listeriosis can cause fever, muscle aches, headache, stiff neck, confusion, loss of balance and convulsions sometimes preceded by diarrhea or other gastrointestinal symptoms. An invasive infection spreads beyond the gastrointestinal tract. In pregnant women, the infection can cause miscarriages, stillbirths, premature delivery or life-threatening infection of the newborn. In addition, serious and sometimes fatal infections in older adults and persons with weakened immune systems. Listeriosis is treated with antibiotics. Persons in the higher-risk categories who experience flu-like symptoms within two months after eating contaminated food should seek medical care and tell the health care provider about eating the contaminated food.
Consumers who have purchased these products are urged not to consume them. These products should be thrown away or returned to the place of purchase.
FSIS routinely conducts recall effectiveness checks to verify recalling firms notify their customers of the recall and that steps are taken to make certain that the product is no longer available to consumers. Lamb Weston is recalling 14 ounce packages of Red Robin Burgers and Brews Crispy Onion Rings manufactured by Fry Foods and sold in grocery stores, because the product may contain an undeclared milk allergen. People who have an allergy or severe sensitivity to milk run the risk of serious or life-threatening allergic reaction if they consume this product.
These products are NOT served in Red Robin restaurants. The Red Robin Burgers and Brews Crispy Onion Rings were distributed in the U.S. through retail grocery stores. No other products are impacted by this recall.
The product comes in a 14 ounce bag and is frozen. Details on the impacted packages are listed below. The Sell By Date and Package Code can be found on the back of the package. The recall was initiated after it was discovered that contract manufacturer Fry Foods mistakenly packed product containing milk in packaging that did not reveal the presence of milk. Subsequent investigation indicates the product did not meet Lamb Weston specifications.
Consumers who have purchased Red Robin Burgers and Brews Crispy Onion Rings from a grocery retailer are urged to return the product to the place of purchase for a full refund.
Fry Foods and Lamb Weston are cooperating with the FDA on this recall and working with customers to ensure packages are removed from store shelves and are no longer distributed.

WASHINGTON, May 11, 2017– NY Livestock Market, Inc., a Brooklyn, N.Y. establishment, is recalling approximately 1,495 pounds of poultry products that may be contaminated with the antibiotic nitrofurazone, the U.S. Department of Agriculture’s Food Safety and Inspection Service (FSIS) announced today.
The whole young chickens were produced and packaged on April 10, 2017. The following products are subject to recall:
  • Brown shipping bags containing various quantities of 4.9-lb. young whole chickens, with head and feet intact, with “NY LIVESTOCK MARKET, INC. BUDDHIST RELIGIOUS EXEMPT (Eviscerated Poultry)” printed on the label.
The products subject to recall bear establishment number “P-45068” inside the USDA mark of inspection. These items were shipped to restaurants and retail locations in New York state.
The problem was discovered through routine testing performed by FSIS inspection personnel for the National Residue Program, which revealed the sample was violative for nitrofurazone, a residue of public health concern.
There have been no confirmed reports of adverse reactions or illnesses due to consumption of these products. Anyone concerned about an injury or illness should contact a healthcare provider.

The Monte Cristo (turkey, ham, cheese, on French toast) sandwiches were produced from May 1, 2014 through February 1, 2017. The following products are subject to recall: [View Label (PDF Only)]
  • 13.25-lb. bulk cases containing 200 “Kabobs MONTE CRISTO TURKEY BREAST, HAM AND CHEESE SANDWICH” in shrink wrapped plastic trays and bearing case codes: PM14E27, PM14H14, PM14J10, PM14E01, PM14E12, PM14E13, PM15C26, PM15E06, PM15G23, PM14L18, PM15A08, PM15C26, PM15K20, PM16B08, PM16B09, PM16F22, PM16F24, and PM17B01.

The problem was discovered when Kabob’s Acquisition, Inc., was notified by their French toast ingredient supplier, that the French toast used in the sandwich products was recalled due to potential Lm contamination. There have been no confirmed reports of adverse reactions due to consumption of these products.

(3 LB.) 1.46 KG Potato Salad because it may contain undeclared egg.  People who have an allergy or severe sensitivity to egg run the risk of serious or life-threatening allergic reaction if they consume these products.
The recalled potato salad was distributed and sold at Wegmans Food Markets in NY, MA and Erie, PA.
The recalled product was packaged in 48 ounce (3 Lb.) clear plastic containers having a label that states Wegmans Family Pack Food You Feel Good About Potato Salad on the lid. The recalled product has a code on the lid that states “SELL BY: 5/7/17 3379-4” and is marked with UPC 77890 24678 on the side label.  
No illnesses have been reported to date.
The recall was initiated after it was discovered that certain packages of the recalled product contained an ingredient and nutrition label from another product [Greek Pasta Feta Salad] that did not reveal the presence of egg.  People who have an allergy or sensitivity to eggs should not consume the potato salad.  If you still have the salad, please return it to the Wegmans store that you purchased it from for a full refund.

Mikawaya is issuing a voluntary recall of a limited number of Mikawaya Brand Chocolate Chocolate Mochi Ice Cream, which may inadvertently contain peanuts. People who have an allergy or severe sensitivity to peanuts run the risk of a serious or life-threatening allergic reaction if they consume the product. Only product labeled with UPC code of 070934990609 and lot number LOT 090-17 is included in this recall. The UPC code can be found on the back of the box and the lot number is printed on the top enclosure.
The affected product is sold in a paperboard box, containing 9.1oz (258g) of 6-1.5oz (43g) pieces with UPC code of 070934990609 and lot number LOT 090-17. The product was distributed to all Trader Joe’s stores nationwide and has been removed from sale.
The recall was initiated after receiving one customer report that a Chocolate Chocolate Mochi Ice Cream contained the presence of peanut butter. Mikawaya has not received any reports of illness or allergic reactions associated with this product.
No other Mikawaya Brand products besides this specific lot of Chocolate Chocolate Mochi Ice Cream are affected by this voluntary recall.
Customers who have purchased Mikawaya Chocolate Chocolate Mochi Ice Cream with the affected UPC code and lot number, and have an allergy or sensitivity to peanuts, are advised not to consume the product. Publix Super Markets is issuing a voluntary recall for Publix Deli Artichoke and Spinach Dip due to the possibility of the product containing small glass fragments.
The 16 oz. artichoke and spinach dip was sold at Publix stores in Florida, Georgia, South Carolina, North Carolina , Alabama and Tennessee with a UPC of  000-41415-15961, and a use-by-date of  May 16 A1 and May 16 C1,which is printed on the lid of the container. The product in question is found in the refrigerated cases in the deli.

Fourth Street Barbecue Inc. Packing Division has initiated a voluntary recall of all lot numbers of the Save-A-Lot Pick 5 Chicken Waffle Sandwich distributed nationwide as it has the potential to be contaminated with Listeria monocytogenes, an organism which can cause serious and sometimes fatal infections in young children, frail or elderly people, and others with weakened immune systems. Although healthy individuals may suffer only short-term symptoms such as high fever, severe headache, stiffness, nausea, abdominal pain and diarrhea, Listeria infection can cause miscarriages and stillbirths among pregnant women.
Product was sold frozen and distributed nationwide to Save-A-Lot stores.

Quality Nut Co. of Massillon, Ohio is voluntarily recalling the following products due to undeclared allergens:
  • SWEET AND SPICY MIX – 2 oz. bags. Labels did not declare the allergens soy and wheat.
