The U.S. Food and Drug Administration, along with the Centers for Disease Control and Prevention (CDC) and state and local partners, are investigating a multistate outbreak of Salmonella Mbandaka illnesses that may be linked to Kellogg’s Honey Smacks cereal.

The FDA is advising consumers to not eat and to discard any Kellogg’s Honey Smacks cereal. This is regardless of size or “best if used by” dates. The recall notice accounts for all of the product that is on the market within the cereal’s estimated one year shelf-life. However, Honey Smacks products with earlier dates could also potentially be contaminated.
The FDA is also disclosing a more detailed list of foreign countries to which the Kellogg’s Honey Smacks cereal was distributed. Here is the list of the foreign countries:
Aruba/Curaçao/Saint Maarten (Netherlands Antilles), the Bahamas, Barbados, Tortola (British Virgin Islands), Costa Rica, Guatemala, Haiti, Mexico, Panama, and Tahiti (French Polynesia).

The FDA and CDC, along with our state partners contacted The Kellogg Company and as a result of discussions, the company has voluntarily recalled Kellogg’s Honey Smacks to prevent further distribution of potentially contaminated products. The recalled products were distributed across the United States including Guam and Saipan, and internationally in certain countries.
There are 73 people ill with this strain of Salmonella in 31 states: Alabama (2), Arizona (1), California (5), Connecticut (3), Georgia (2), Illinois (1), Indiana (3), Kentucky (1), Louisiana (2), Massachusetts (5), Maryland (1), Michigan (4), Mississippi (1), Montana (1), North Carolina (3), New Hampshire (1), New Jersey (3), New York (7), Ohio (1), Oklahoma (2), Oregon (1), Pennsylvania (5), Rhode Island (2), South Carolina (1), Tennessee (1), Texas (2), Utah (1), Virginia (4), Washington (3), Wisconsin (1), West Virginia (3).The ages of the ill people range from less than one year to 87 (median 58 years) and 65% of cases are female. Reported illness onset dates range from 3/3/18 – 5/28/18. Among 55 with available information, 24 (44%) have been hospitalized. No deaths have been reported.

What are the Symptoms of Salmonella Infections? 

Most people infected with Salmonella develop diarrhea, fever, and abdominal cramps. The illness usually lasts 4 to 7 days, and most people recover without treatment. Most people infected with Salmonella develop diarrhea, fever, and abdominal cramps 12 to 72 hours after infection.
In some people, the diarrhea may be so severe that they need to be hospitalized. In these patients, the Salmonellainfection may spread from the intestines to the blood stream and then to other body sites and can cause death unless the person is treated promptly with antibiotics.

Who is at Risk?

Children are the most likely to get salmonellosis. The rate of diagnosed infections in children less than five years old is higher than the rate in all other people. Children younger than five, the elderly, and those people with weakened immune systems are more likely to have severe infections. It is estimated that approximately 400 persons in the United States die each year with acute salmonellosis. 

What Do Restaurants and Retailers Need To Do?

Retailers should not sell and should discard all recalled products. The recalled products were distributed across the United States including Guam and Saipan, and internationally in: 
Aruba/Curaçao/Saint Maarten (Netherlands Antilles), the Bahamas, Barbados, Tortola (British Virgin Islands), Costa Rica, Guatemala, Haiti, Mexico, Panama, and Tahiti (French Polynesia)
Retailers, restaurants, and other food service operators should always practice safe food handling and preparation measures. It is recommended that they wash hands, utensils, and surfaces with hot, soapy water before and after handling food.
  • Wash and sanitize display cases and refrigerators regularly.
  • Wash and sanitize cutting boards, surfaces, and utensils used to prepare, serve, or store food.
  • Wash hands with hot water and soap following the cleaning and sanitation process.
  • Regular frequent cleaning and sanitizing of food contact surfaces and utensils used in food preparation may help to minimize the likelihood of cross-contamination.

What Do Consumers Need To Do?

The FDA is advising consumers to not eat and discard any Kellogg’s Honey Smacks cereal. This is regardless of size or “best if used by” dates. If already purchased, consumers should throw it away or return to the place of purchase for a refund. The FDA continues to collect information to determine any additional sources. Initial Announcement

June 14, 2018

CDC, public health and regulatory officials in several states, and the U.S. Food and Drug Administration are investigating a multistate outbreak of Salmonella Mbandaka infections.
Public health investigators are using the PulseNet system to identify illnesses that may be part of this outbreak. PulseNet is the national subtyping network of public health and food regulatory agency laboratories coordinated by CDC. DNA fingerprinting is performed on Salmonella bacteria isolated from ill people by using techniques called pulsed-field gel electrophoresis (PFGE) and whole genome sequencing (WGS). CDC PulseNet manages a national database of these DNA fingerprints to identify possible outbreaks. WGS gives a more detailed DNA fingerprint than PFGE.
As of June 14, 2018, 73 people infected with the outbreak strain of Salmonella Mbandaka have been reported from 31 states. A list of the states and the number of cases in each can be found on the Case Count Map page.
Illnesses started on dates from March 3, 2018, to May 28, 2018. Ill people range in age from less than one year to 87, with a median age of 58. Sixty-five percent are female. Out of 55 people with information available, 24 (44%) have been hospitalized. No deaths have been reported.
Illnesses that occurred after May 22, 2018, might not yet be reported due to the time it takes between when a person becomes ill and when the illness is reported. This takes an average of 2 to 4 weeks. Please see the Timeline for Reporting Cases of SalmonellaInfection for more details.

Investigation of the Outbreak

Epidemiologic evidence indicates that Kellogg’s Honey Smacks cereal is a likely source of this multistate outbreak.
In interviews, ill people answered questions about the foods they ate and other exposuresin the week before they became ill. Thirty (77%) of 39 people interviewed reported eating cold cereal. In interviews, 14 people specifically reported eating Kellogg’s Honey Smacks cereal. Ill people in this outbreak reported this cereal more often than any other cereals or food items.
On June 14, 2018, the Kellogg Company recalled 15.3 oz. and 23 oz. packages of Kellogg’s Honey Smacks cereal.

WASHINGTON, June 14, 2018 – Winter Sausage Manufacturing Co, an Eastpointe, Mich. establishment, is recalling approximately 28,346 pounds of poultry, pork, and beef products due to misbranding, the U.S. Department of Agriculture’s Food Safety and Inspection Service (FSIS) announced today. The poultry products were slaughtered under religious exemption, which is not declared on the product label.
The poultry, pork, and beef items were produced on various dates from March 9, 2018 through June 4, 2018. The following products that are subject to recall can be found here. [View Labels (PDF only)]
The products subject to recall bear establishment number “P-10158” inside the USDA mark of inspection.
These items were shipped to retail locations and distributors in Illinois, Michigan and Pennsylvania.
The problem was discovered on June 12, 2018 by FSIS during routine labeling verification.  
There have been no confirmed reports of adverse reactions due to consumption of these products. Anyone concerned about an injury or illness should contact a healthcare provider.  
FSIS is concerned that some product may be frozen and in consumers’ freezers. Consumers who have purchased these products are urged not to consume them. These products should be thrown away or returned to the place of purchase.
FSIS routinely conducts recall effectiveness checks to verify that recalling firms are notifying their customers of the recall and that actions are being taken to make certain that the product is no longer available to consumers.

On June 1, 2018, the FDA learned of the Salmonella Adelaide infections currently being investigated in multiple states that are associated with exposure to fruit salad mixes that include pre-cut melons.

What are the Symptoms of Salmonella Infections?

Most people infected with Salmonella develop diarrhea, fever, and abdominal cramps. The illness usually lasts 4 to 7 days, and most people recover without treatment. Most people infected with Salmonella develop diarrhea, fever, and abdominal cramps 12 to 72 hours after infection.
In some people, the diarrhea may be so severe that they need to be hospitalized. In these patients, the Salmonellainfection may spread from the intestines to the blood stream and then to other body sites and can cause death unless the person is treated promptly with antibiotics.

Who is at Risk?

Children are the most likely to get salmonellosis. The rate of diagnosed infections in children less than five years old is higher than the rate in all other people. Children younger than five, the elderly, and those people with weakened immune systems are more likely to have severe infections. It is estimated that approximately 400 persons in the United States die each year with acute salmonellosis. .  

What Do Restaurants and Retailers Need To Do?

Retailers should not sell and should discard all recalled products listed above that were supplied by Caito Foods, LLC. The products were packaged in clear, plastic, clamshell containers and distributed in Alabama, California, Florida, Georgia, Illinois, Indiana, Iowa, Kansas, Kentucky, Maryland, Michigan, Minnesota, Missouri, Nebraska, North Carolina, North Dakota, Ohio, Pennsylvania, South Dakota, Tennessee, Virginia, West Virginia, and Wisconsin. . Retailers, restaurants, and other food service operators should always practice safe food handling and preparation measures. It is recommended that they wash hands, utensils, and surfaces with hot, soapy water before and after handling food.
  • Wash and sanitize display cases and refrigerators regularly.
  • Wash and sanitize cutting boards, surfaces, and utensils used to prepare, serve, or store food.
  • Wash hands with hot water and soap following the cleaning and sanitation process.
  • Regular frequent cleaning and sanitizing of food contact surfaces and utensils used in food preparation may help to minimize the likelihood of cross-contamination.  

What Do Consumers Need To Do?

Consumers should not eat any pre-cut melon or fruit salad mixes containing pre-cut melons that were recalled. The FDA is advising consumers to view the list of retailers and recalled products. If already purchased, consumers are advised to throw away the recalled pre-cut melons or return to the place of purchase for a refund. The FDA continues to collect information to determine any additional sources. The FDA will update this posting as soon as more information becomes available.
Consumers should always practice safe food handling and preparation measures. It is recommended that they wash hands, utensils, and surfaces with hot, soapy water before and after handling food.

World Variety Produce, Inc. of Los Angeles, CA is voluntarily recalling Spicy Edamame 7oz, because it contains the undeclared shellfish/crustacean allergen “Oyster Extract” ingredient within the spicy sauce packet. People who have an allergy or severe sensitivity to shellfish run the risk of serious or life-threatening allergic reaction if they consume these products.
Spicy Edamame were distributed through retailers in Arizona, California, Georgia, Indiana, Louisiana, New Mexico, Nevada, Ohio, Pennsylvania, and Texas.

WASHINGTON, June 13, 2018 – Venda Ravioli, Inc., a Providence, R.I. establishment, is recalling approximately 738 pounds of frozen meat ravioli products that were produced without the benefit of federal inspection while the establishment was under suspension, the U.S. Department of Agriculture’s Food Safety and Inspection Service (FSIS) announced today.
The scope of this recall expansion now includes an additional 66 pounds of frozen meat and poultry pasta products, which were produced and packed from Feb. 22, 2018 through June 7, 2018. [View Labels (PDF only)]  
  • 16 oz. sealed trays of “VENDA RAVIOLI, PASTA FRESCA, COSTANTINO’S, Chicken Prosciutto Ravioli,” and a lot code of 05318-15918.
  • 16 oz. sealed trays of “VENDA RAVIOLI, PASTA FRESCA, COSTANTINO’S, Chicken Prosciutto Walnuts Ravioli,” and a lot code of 05318-15918.

The problem was discovered on June 12, 2018, while FSIS was performing recall effectiveness checks. The items were shipped to retail locations in Rhode Island.
On June 8, 2018, Venda Ravioli, Inc. recalled approximately 672 pounds of frozen meat ravioli items that were produced and packaged on various dates from Feb. 22, 2018 through June 7, 2018. The following products are subject to recall: 
  • 16 oz. sealed trays of “VENDA RAVIOLI, PASTA FRESCA, COSTANTINO’S, MEAT RAVIOLI,” and lot codes of 05318, 05418, 05518, 05618, 05718, 05818, and 05918. 
  • 16 oz. sealed trays of “VENDA RAVIOLI, PASTA FRESCA, COSTANTINO’S, Italian Sausage Ravioli.”
  • 16 oz. sealed trays of “VENDA RAVIOLI, PASTA FRESCA, COSTANTINO’S, Prosciutto & Provolone Ravioli,” and lot codes of 05318, 05418, 05518, 05618, 05718, 05818, and 05918.
  • 16 oz. sealed trays of “VENDA RAVIOLI, PASTA FRESCA, COSTANTINO’S, OSSO BUCO RAVIOLI.”
The Meat Ravioli and Prosciutto & Provolone and Chicken Prosciutto Ravioli products subject to recall bear establishment number “EST.9501” inside the USDA mark of inspection. The Italian Sausage Ravioli, Osso Bucco Ravioli, Chicken Tortelloni and Chicken Prosciutto Walnuts Ravioli products do not have the USDA mark of inspection on the packaging. These items were shipped to retail locations in Massachusetts and Rhode Island.                     
There have been no confirmed reports of adverse reactions due to consumption of these products. Anyone concerned about a reaction should contact a healthcare provider.  
FSIS is concerned that some product may be in consumers’ freezers. Consumers who have purchased these products are urged not to consume them. These products should be thrown away or returned to the place of purchase.
FSIS routinely conducts recall effectiveness checks to verify recalling firms notify their customers of the recall and that steps are taken to make certain that the product is no longer available to consumers. WASHINGTON, June 13, 2018 – HF Food Distributors, a Orocovis, Puerto Rico establishment, is recalling approximately 142 pounds of sliced cooked ham products that were produced without the benefit of federal inspection, the U.S. Department of Agriculture’s Food Safety and Inspection Service (FSIS) announced today.
The ready-to-eat sliced cooked ham products were produced and packed on April 21, 2018. The following products are subject to recall: [View Labels (PDF only)]   
  • 1-lb. plastic-wrapped packages of “JAMON COCIDO REBANADO” with a sell by date of “May 25, 00” on the label.
The products do not bear an establishment number, mark of inspection, or an ingredients statement. These items were shipped wholesale to Head Start centers in Puerto Rico.  USDA provides reimbursement for meals served in Head Start through the Child and Adult Care Food Program; however, these products were purchased commercially.  The products were not provided or purchased by USDA’s Food and Nutrition Service.           
The problem was discovered on June 8, 2018, when FSIS performed recall effectiveness checks for FSIS Recall 044-2018.
There have been no confirmed reports of adverse reactions due to consumption of these products. Anyone concerned about a reaction should contact a healthcare provider.  
FSIS is concerned that some product may be frozen and in consumers’ freezers. Consumers who have purchased these products are urged not to consume them. These products should be thrown away or returned to the place of purchase.
FSIS routinely conducts recall effectiveness checks to verify recalling firms notify their customers of the recall and that steps are taken to make certain that the product is no longer available to consumers. When available, the retail distribution list(s) will be posted on the FSIS website at www.fsis.usda.gov/recalls.

Dave’s Pet Food of Agawam, MA is recalling a single lot of Dave’s Dog Food 95% premium beef cans because the products potentially contain elevated levels of beef thyroid hormone.
Product was distributed all along the east coast of the US, sold in pet stores and ecommerce sites.
The recalled product consists of a single batch (548 cases) of 13 oz., 95% premium beef dog food with a UPC # of 85038-11167 and a date code of 08/2020.
Dogs consuming high levels of beef thyroid hormone may exhibit symptoms such as increased thirst and urination, weight loss, increased heart rate and restlessness. These symptoms may resolve when the consumption of these levels is discontinued. However, with prolonged consumption these symptoms may increase in severity and may include vomiting, diarrhea, and rapid or labored breathing. Should these symptoms occur, we recommend pet owners contact their veterinarian immediately.
The recall was initiated after FDA informed Dave’s that one lot of product was analyzed and found to have elevated levels of thyroid hormone. FDA analyzed the product after receiving a complaint that four dogs consuming it were found to have low Free T4 (fT4) and Thyroid stimulating hormone (TSH).
Consumers who have purchased the specific product listed above should stop feeding it to their dogs. If consumers have questions or would like to receive a refund or coupon for replacement product, they should call us at 1-888-763-2738 Monday through Friday, between 9:00 AM and 5:00 PM EST.
No other Dave’s products, or any other product manufactured by Dave’s Pet Food, are impacted.
The voluntary recall is being conducted in cooperation with the U.S. Food and Drug Administration.

WASHINGTON, June 12, 2018 – J Bar B Foods, a Waelder, Texas establishment, is recalling approximately 410,985 pounds of beef brisket smoked sausage products due to an adulterated and misbranded ingredient, the U.S. Department of Agriculture’s Food Safety and Inspection Service (FSIS) announced today. The products contain a pork casing, which is not declared on the product label, and may cause an allergic reaction in some individuals.
The ready-to-eat beef brisket smoked sausage products were produced on various dates from June 21, 2016 to May 29, 2018. The following products are subject to recall: [View Label (PDF only)]
  • 12 oz. vacuum-packed packages of “H-E-B TEXAS HERITAGE SAUSAGE SMOKED WITH NATURAL HARDWOOD BEEF BRISKET.”
Lot codes, packaging dates and other identifying label information may be found here.
The products subject to recall bear establishment number “EST. 7066” inside the USDA mark of inspection. These items were shipped to retail locations in Texas.
The problem was discovered on June 12, 2018 in response to a customer inquiry that a beef smoked sausage product did not specify pork casing as part of the ingredients statement.

Pre-Opening review: Your Pie, Flagstaff, AZ

Today I was invited by the franchise owner of the new, soon to be opened, Your Pie, here in Flagstaff, AZ, for a special Dine & Donate event (soft opening) to benefit the Coconino County Humane Association.

I arrived a bit early to get my bearings and to see how things were progressing. The owner had discovered I have a background in restaurant operations, so I have been answering questions and offering suggestions as I have seen them. Lisa is a wonderful woman and a joy to watch as she makes working in a restaurant fun and exciting.

The concept of the restaurant is very easy to understand. You can purchase a 10″ pizza made the way you want it, with unlimited toppings for $8.75 plus tax. the menu boards are actually displayed from right to left, as you walk past them; however, I am placing them from left to right as that is how we actually read things. Also had to take the pictures at an angle because the light glared on the menu boards. Check this out:

I got to be the first one in line during the dine and donate promotion, and I ordered my pizza, white crust, marinara sauce, shredded mozzarella, pepperoni, mushrooms, and jalapeno’s. I also ordered an order of cheese sticks with marinara, and a soda in a souvenir cup (the cup allows for discounted drinks in the future). I paid for my meal, went to get my soda. by the time I sat down, my food was being delivered. Throughout the day, I watched as food was being delivered either by the time the guests were sitting down or shortly thereafter. The pizza and the cheese sticks were very good, although I do prefer warm marinara to be served with cheese sticks (Lisa said she likes that as well and will be working on it for the future). I will definitely be visiting this location in the future, especially this Friday for the Grand Opening celebration. I will probably order my already favorite meal.
