WASHINGTON, July 12, 2016 – Simmons Prepared Foods, Inc., a Van Buren, Ark. establishment, is recalling approximately 5,850 pounds of frozen, heat treated, not ready-to-eat (NRTE) chicken products that may be contaminated with E. coli O121, the U.S. Department of Agriculture’s Food Safety and Inspection Service (FSIS) announced today.
The frozen, heat treated, not ready-to-eat (NRTE) chicken tenderloin items were produced on Jan. 25, 2016. The following products are subject to recall: [View Labels (PDF Only)]
  • 30-lb. net-weight case containing six, 5-lb. bags in clear film of “Simmons UNCOOKED CHICKEN TENDERLOIN FRITTERS,” with a case code 31473, packaging date code of 6025, and a Use-By date of 01/25/17.
  • 30-lb. net-weight case containing six, 5-lb. bags in clear film of “Simmons UNCOOKED CHICKEN BREAST TENDERLOIN FRITTERS,” with a case code 62331 and a packaging date of 6025.
The products subject to recall bear establishment number “P-5837” inside the USDA mark of inspection. These items were distributed to Arkansas for institutional use.
The problem was discovered on July 7, 2016, when Simmons Prepared Foods, Inc. received notice from a supplier that flour sold to the establishment was recalled by General Mills. The firm used the recalled flour to bread the chicken fritters affected by this recall action. There have been no confirmed reports of adverse reactions or illnesses due to consumption of these Simmons Prepared Foods, Inc. products.

Many clinical laboratories do not test for non-O157 Shiga toxin-producing E. coli (STEC), such as STEC O121 because it is harder to identify than STEC O157. People can become ill from STECs 2–8 days (average of 3–4 days) after consuming the organism. Most people infected with STEC O121 develop diarrhea (often bloody), and vomiting. Some illnesses last longer and can be more severe. Infection is usually diagnosed by testing of a stool sample. Vigorous rehydration and other supportive care is the usual treatment; antibiotic treatment is generally not recommended.
Most people recover within a week, but, rarely, some develop a more severe infection. Hemolytic uremic syndrome (HUS) is uncommon with STEC O121 infection. HUS can occur in people of any age but is most common in children under 5 years old, older adults and persons with weakened immune systems. It is marked by easy bruising, pallor, and decreased urine output. Persons who experience these symptoms should seek emergency medical care immediately.
FSIS and the company are concerned that some product may be frozen and in consumers’ freezers.
Consumers who have purchased these products are urged not to consume them. These products should be thrown away or returned to the place of purchase.
FSIS routinely conducts recall effectiveness checks to verify recalling firms notify their customers of the recall and that steps are taken to make certain that the product is no longer available to consumers. The Kroger Co. (NYSE: KR) has recalled Kroger Deluxe S’mores Ice Cream (48 oz, tub) sold in its retail stores because the product may contain peanuts not listed on the label.  Kroger initiated the recall when a supplier, Grain Craft, indicated a raw ingredient may have become contaminated with low levels of peanut residue.
People who are allergic to peanuts could have a severe reaction if they consume this product. For consumers who are not allergic to peanuts, there is no safety issue with the product. No customer illnesses have been reported at this time.
All stores operating under the Kroger, Dillons, Baker’s, Gerbes, Food 4 Less, Fred Meyer, Fry’s, JayC, King Soopers, City Market, QFC, Ralphs and Smith’s names, stores located in Alaska, Alabama, Arkansas, Arizona, California, Colorado, Georgia, Idaho, Illinois, Indiana, Kansas, Kentucky, Michigan, Missouri, Mississippi, Montana, North Carolina, Nebraska, New Mexico, Nevada, Ohio, Oregon, South Carolina, Tennessee, Utah, Virginia, Washington, West Virginia, and Wyoming no other Kroger locations in Louisiana or Texas are included.

Continental Mills has issued a recall affecting only retail Krusteaz Blueberry Pancake Mix, which involves product manufactured between April 2016 and June 2016.  The company was notified by our supplier that their product, a blueberry nugget, is made with a small percentage of affected flour which was recalled by General Mills because it may be contaminated with E. coli O121.  This is an isolated issue, and only affects specific lots of Krusteaz Blueberry Pancake Mix.  Food Safety is our highest priority and this has caused us to take action for the safety of our consumers.
Most strains of E. coli are harmless, however, others can make you sick. E. coli O121 is a potentially deadly bacterium that can cause bloody diarrhea and dehydration. People who experience these symptoms should seek emergency medical care immediately. The very young, seniors, and those with compromised immune systems are the most susceptible to foodborne illness.  Anyone diagnosed by a physician as having an illness related to E. coli 0121 should contact state and local public health authorities.
No illnesses have been reported to date.
FDA and the Centers for Disease Control (CDC) continues to warn that consumers should refrain from consuming any raw products made with flour. E. coli O121 is eliminated by heat through baking, frying, sautéing or boiling products made with flour. All surfaces, hands and utensils should be properly cleaned after contact with flour or dough.
Product was distributed nationwide where consumers purchased product through retail stores.

The problem was discovered when a supplier notified Kabob’s Acquisition, Inc. that flour sold to the establishment was recalled by General Mills due to possible association with a multi-state Escherichia coli O121 illness outbreak. There have been no confirmed reports of adverse reactions due to consumption of these Kabob’s Acquisition, Inc. products.

Hearn Kirkwood is initiating a voluntary recall of “Evie’s Cheddar Potato Salad” because it has the potential to be contaminated with Listeria monocytogenes, an organism which can cause serious and sometimes fatal infections in young children, frail or elderly people, and others with weakened immune systems. Although healthy individuals may suffer only short-term symptoms such as high fever, severe headache, stiffness, nausea, abdominal pain and diarrhea, Listeriainfection can cause miscarriages and stillbirths among pregnant women.
Hearn Kirkwood is issuing this voluntary recall after being notified by their supplier, National Frozen Foods, that the frozen green peas used in the product are being recalled because they have the potential to be contaminated with Listeria monocytogenes.
The recalled products was sold through Amazon Fresh in the locations Bellevue, Washington area between 04/08/16 and 06/13/16.
There have been no reported illnesses attributed to the recalled items to date.
The recalled product was sold in 6.0 oz. clear plastic containers with the name “Evie’s Cheddar Potato Macaroni Salad,” UPC code 6637511772” and “use by” dates of 04/15/16 to 06/20/16.
Consumers who have purchased this product should discard it and contact Amazon Fresh at (866) 380-0525 between the hours of 6:00 AM – 10:00 PM EST for a full refund.

Mister Cookie Face LLC, a subsidiary of Fieldbrook Foods Corporation, has issued a voluntary recall of Sundae Shop Novelty Ice Cream Dipped “Variety” Cones due to the possibility of peanuts on the product that are not declared on the allergen statement.
Individuals who have an allergy or severe sensitivity to peanuts run the risk of serious or life-threatening allergic reaction if they consume products containing peanut allergens.
The recalled product with the best before date November 14, 2017 may actually contain a Chocolate covered Vanilla Ice cream cone with peanut sprinkles on top. The label of the Sundae Shop Novelty Ice Cream Dipped “Variety” Cones does not list peanuts as an ingredient.
The affected product is packaged in a 32 oz. box containing eight ice cream cones, has the UPC code: 041498191962, and the plant code 34-2036, which is located next to the “best by” date.
The recalled Sundae Shoppe Ice Cream Dipped “Variety” Cones were distributed only through ALDI Inc. ALDI representatives worked with Mister Cookie Face to ensure any incorrectly packaged product was removed from the store shelves. No other Mister Cookie Face or ALDI products are affected by this recall.
Mister Cookie Face LLC and ALDI take the safety and integrity of their products seriously. If customers have product affected by this voluntary recall, they should discard it immediately or return it to their local store for a full refund.

Cumberland Farms announces a voluntary recall of its SEA SALT CARAMEL DELIGHTS flavor of Cumberland Farms Farmhouse Premium Chocolate Treats due to concerns of the possible presence of peanuts.  People who have an allergy or severe sensitivity to specific types of allergen, such as peanuts, run the risk of serious life threatening allergic reactions if they consume the product being recalled.
While the product line is available in multiple flavors, the only product being recalled is the SEA SALT CARAMEL DELIGHTS flavor of Cumberland Farms Farmhouse Premium Chocolate Treats which is only sold in 3-ounce bags.  Although this is believed to be an extremely isolated incident, effective immediately, all Cumberland Farms retail locations have withdrawn this product from sale.  This recall does not include any other flavor of Cumberland Farms Premium Chocolate Treats or Farmhouse Premium Chocolate Bars. 
Although the packaging clearly states the product is packed in a facility where other products containing peanuts, tree nuts, milk, wheat, soy and eggs are packaged, out of an abundance of caution, Cumberland Farms is recalling all of the three-ounce bags of the Cumberland Farms Farmhouse Premium Chocolate Treats SEA SALT CARAMEL DELIGHTS.
We were made aware of the presence of the peanuts through two customer complaints. Cumberland Farms is not aware of any related illnesses at this time.
Cumberland Farms is asking that customers who purchased the affected product discard it immediately or return the product to the nearest Cumberland Farms retail location for a full refund.

The ready-to-eat, Lifestyle Foods Inc. brand of Lemongrass Basil Chicken product was produced on June 13 and June 14, 2016. The following product is subject to recall: [View Labels (PDF Only)] 
  • 8.0-oz. (227g) individually wrapped entrée packages labeled “Lifestyle Foods Lemongrass Basil Chicken Entrée – Brown Rice in a Thai Style Sauce with Grilled Chicken and Green Beans.” The product has “Enjoy By” dates of June 28, 2016 and June 29, 2016.

The problem was discovered when Winter Gardens Quality Foods, Inc. notified FSIS that a component of the product had tested positive for Listeria monocytogenes. There have been no confirmed reports of illness or adverse reactions due to consumption of these products.
Consumption of food contaminated with Listeria monocytogenes can cause listeriosis, a serious infection that primarily affects older adults, persons with weakened immune systems, and pregnant women and their newborns. Less commonly, persons outside these risk groups are affected.
Listeriosis can cause fever, muscle aches, headache, stiff neck, confusion, loss of balance and convulsions sometimes preceded by diarrhea or other gastrointestinal symptoms. An invasive infection spreads beyond the gastrointestinal tract. In pregnant women, the infection can cause miscarriages, stillbirths, premature delivery or life-threatening infection of the newborn. In addition, serious and sometimes fatal infections in older adults and persons with weakened immune systems. Listeriosis is treated with antibiotics. Persons in the higher-risk categories who experience flu-like symptoms within two months after eating contaminated food should seek medical care and tell the health care provider about eating the contaminated food.
Consumers who have purchased these products are urged not to consume them. These products should be thrown away or returned to the place of purchase.
FSIS routinely conducts recall effectiveness checks to verify recalling firms notify their customers of the recall and that steps are taken to make certain that the product is no longer available to consumers. When available, the retail distribution list(s) will be posted on the FSIS website at www.fsis.usda.gov/recalls.
FSIS advises all consumers to reheat ready-to-eat product until steaming hot.

WASHINGTON, July 1, 2016 – GHSE, LLC, a Green Cove Springs, Fla. establishment, is recalling approximately 5,682 pounds of salad products containing chicken due to misbranding and an undeclared allergen, the U.S. Department of Agriculture’s Food Safety and Inspection Service (FSIS) announced today. The product contains wheat, a known allergen not declared on the product label.
The ready-to-eat salads with chili-seasoned chicken items were produced on various dates between May 11, 2016 and June 29, 2016. The following products are subject to recall: [View Labels (PDF Only)]
  • 11-oz. plastic clam shell containers with “TRADER JOSE’S MEXICALI INSPIRED SALAD WITH CHILI SEASONED CHICKEN” with “BEST BY” dates 05/18/16 – 07/03/16.
The products subject to recall bear establishment number “EST. P-45781” inside the USDA mark of inspection. These items were shipped to retail locations in Alabama, Florida, Georgia, North Carolina, South Carolina, Tennessee, and Southern Virginia.                           
The problem was discovered by a plant quality assurance technician during a routine check of ingredients. The salads have the correct label on the top of the package, but exhibit an incorrect back label that does not include soy sauce as an ingredient; the source of the wheat. As a result, wheat is not sub-listed in the ingredients statement or in the “Contains” allergen declaration.
There have been no confirmed reports of adverse reactions due to consumption of these products. Anyone concerned about an injury or illness should contact a healthcare provider.

General Mills of Minneapolis, MN, continues to collaborate with health officials to investigate a multistate outbreak of E. coli O121, and is expanding its recall of Gold Medal flour, Wondra flour, and Signature Kitchens flour to include flour made earlier in the fall that may still be in consumers’ pantries. The recall is being expanded due to a newly-reported illness that appears to have stemmed from the consumption of raw dough or batter linked to flour produced last fall.
Guidance from the Food and Drug Administration (FDA) and the Centers for Disease Control (CDC) continues to warn that consumers should refrain from consuming any raw products made with flour. E. coli O121 is eliminated by heat through baking, frying, sautéing or boiling products made with flour. All surfaces, hands and utensils should be properly cleaned after contact with flour or dough.
Although most strains of E. coli are harmless, others can make you sick. E. coliO121 is a potentially deadly bacterium that can cause bloody diarrhea and dehydration. Seniors, the very young, and persons with compromised immune systems are the most susceptible to foodborne illness.
Anyone diagnosed by a physician as having an illness related to E. coli O121 is also urged to contact state and local public health authorities.
The recall affects the following retail flour products. It includes 8 SKUs (stock keeping units or UPC codes) of Gold Medal Flour, 1 SKU of Signature Kitchens Flour and 1 SKU of Gold Medal Wondra flour.
